• Recognize and describe the Top 10 error traps or precursors
• Develop a means to recognize individual error traps that are present at work and off-the-job
• Learn to recognize error traps and how they impact performance
• Recognize the various types of error prevention tools and when to apply them to help reduce and prevent errors from occurring
• Practice applying prevention tools that can be used at work and off-the-job
HOP-Based Cause Analysis
This workshop covers HOP in relation to Cause Analysis with the following desired outcomes:
Objective: Our Basic Cause Analysis Workshop improves the capabilities of individuals to perform effective cause analysis for minor incidents and identify corrective actions capable of reducing the risk of recurrence.
Duration and Delivery Format: 3 - 4 Hours (self-paced) - Online Module with Interactive Quizzes
Content: Attendees will learn
• Definitions for terms relevant to cause analysis
• How and Why incidents occur
• Phases of the Cause Analysis Process
• How to determine the cause(s) of an incident
• How to assess the incident’s broader ramifications (Extent of Condition)
• How to identify a corrective action plan that effectively resolves the incident
HOP-Based Procedure & Process Writing/Reviewing
This workshop covers HOP in relation to Procedure & Process Writing/Reviewing with the following desired outcomes:
• Familiarize procedure writers with human performance fundamentals, and the impact procedures have on performance
• Familiarize procedure writers with the common error traps that are put in written guidance
• Provide procedure writers with a standardized writers guide on which to base error-free procedures
• Provide procedure writers with a flow for procedure development that takes the potential error traps into consideration
• Practice using the knowledge on procedures the writers are currently working on
