Objective:

Our Cause Analysis Process Overview course equips Senior Leaders and Managers to understand and fulfill their key role in ensuring effective analyses are completed and optimal corrective action plans are implemented following incidents.

• How and Why incidents occur, including the dominant role organizational issues play in driving the human behaviors and equipment failures that produce incidents

• Roles and responsibilities associated with cause analysis processes (Senior Leaders, Line Management, Issue Manager, Team Leader, Team Member)

• An overview of cause analysis processes and tools

• Approaches for determining an incident’s broader ramifications (Extent of Condition & Extent of Cause)

• Methods and tools for identifying corrective action plans capable of permanently reducing the risk of incident recurrence to acceptable levels (cost-effective surgical strikes, as opposed to inefficient shotgun approaches that attempt to address everything)

• Critical-to-Quality elements that Senior Leaders and Managers should look for when assessing the quality of cause analysis reports

Root Cause Analysis Workshop

Objective:

Our Root Cause Analysis (RCA) Workshop improves the capabilities of individuals to determine the causes of incidents and identify optimal solutions that either prevent recurrence or reduce the risk to acceptable levels.

Duration and Delivery Format:

2 Days – Classroom

Content:

Attendees are taught Human and Organizational Performance (HOP) fundamentals, incident analysis techniques, and corrective action plan development approaches.

Topics include:

• HOP fundamentals and their application to incident analysis

• Proven processes for performing efficient and effective incident analysis

• Interviewing skills

• Four analytical techniques and their proper application

• How to identify an incident’s causal and contributing factors

• Methods for determining an incident’s broader ramifications (Extent of Condition / Extent of Cause)

• How to identify and evaluate prior occurrences for relevance to the current incident

• A structured approach for developing lean and effective corrective action plans

• Principles for writing effective incident analysis reports

Activities:

Attendees will complete an RCA on an interesting case study and perform the following hands-on experiential learning activities:

Objective:

The Advanced Root Cause Analysis (RCA) Workshop is tailored for RCA Team Leaders, experienced analysts, and individuals responsible for their company’s incident analysis program. Our workshop improves the capabilities of individuals to effectively lead incident analysis teams, determine the causes of incidents, and identify optimal solutions that either prevent recurrence or reduce the risk to acceptable levels.

Duration and Delivery Format:

4 Days – Classroom

Attendees are taught proven project management approaches for leading incident analysis teams, as well as provided advanced training in Human and Organizational Performance (HOP) fundamentals, incident analysis techniques, and corrective action plan development approaches. Topics include:

• How to effectively manage incident analysis teams and ensure successful outcomes

• HOP fundamentals, including the impact of personality on individual and team performance

• Proven processes for performing efficient and effective incident analysis

• Interviewing skills

• Eight analytical techniques and their application to single and multi-incident cause analysis

• How to identify an incident’s causal and contributing factors

• Methods for determining an incident’s broader ramifications (Extent of Condition / Extent of Cause)

• How to identify and evaluate prior occurrences for relevance to the current incident

• A structured approach for developing lean and effective corrective action plans

• Principles for writing effective incident analysis reports
