Rick led incident analysis teams in the commercial nuclear power industry for over two decades prior to joining FIT, and has trained Incident Analysts in numerous countries. He was among the first to integrate Human Performance principles into Incident Analysis approaches, and successfully managed the Corrective Action Program for a major corporation. Rick is a co-author of IEEE Standard 1707-2015, Recommended Practice for the Investigation of Events at Nuclear Facilities. Rick, a veteran of the US Navy submarine service, enjoys outdoor activities and resides with his family in Kent, Ohio.

Our workshop equips individuals seeking initial training as minor incident analysts, analysts who are already highly experienced, and individuals who lead minor incident analysis teams, with knowledge and skills that enable performing more efficient and effective limited scope (apparent cause) investigations.  Managers who provide oversight for incident analysis programs should also attend.

Our workshop is intended to improve the capability of organizations to solve moderately significant incidents, thereby improving safety, quality and productivity.
Our workshop integrates human performance fundamentals with proven incident analysis processes and techniques.  Attendees are taught how to perform various analytical techniques and how use of those techniques will lead them to the causes of events, thereby facilitating identification and implementation of corrective actions capable of reducing the risk of recurrence.  Topics include:


  • Human performance fundamentals from an incident analysis perspective
  • Applying proven incident analysis processes, tools, and techniques
  • Structured approach for determining the Extent of Condition
  • Identifying causal factors that induce undesirable human behaviors and equipment failures
  • Identifying contributing factors that allow incidents to occur and/or increase their significance
  • Principles for developing lean and effective corrective action plans
